The year 6 transition form is an online spreadsheet for Liverpool Schools to complete for pupils going to secondary school that year. The form is pre populated so that year 6 teachers and admin/pastoral staff can more readily complete required data for the secondary schools. This form has been designed to reduce the workload of staff and ensure that all schools receive the same information about each child. The data is managed by Angel Solutions and securely sent to schools in the summer term. LLP coordinates this work on behalf of the schools in the city of Liverpool.
There is also a transition form for pupils moving from Infant to Junior school and transition documents and forms for those moving from EYFS settings into Reception. In time, we plan to move all of these forms online for ease of use by schools and early years settings.
